Titre de la thèse : La trajectoire française du traitement hormonal de la ménopause (1950 – 2020) : entre médicalisation et démédicalisation

Le traitement hormonal de la ménopause est une pratique médicale dont la trajectoire a connu des phases d'essor et des crises de remise en cause de son utilité. Diffusé à partir des années 1960, il bat en brèche comme la pilule contraceptive sous les critiques du féminisme américain dans la décennie suivante, avant de resurgir via une nouvelle formulation et de nouveaux arguments médicaux dans les années 80 et 90. Prescrit à presqu'une femme ménopausée sur deux au tournant du siècle aux Etats-Unis, la crise de 2002 liée aux résultats de la Women's Health Initiative entraîne des réécritures des recommandations émanant des divers interlocuteurs en santé publique et une baisse transnationale et durable de sa consommation. Ce sujet propose, à l'image des travaux sur l'histoire française de la contraception en lien avec l'organisation de la gynécologie, et à partir d'archives industrielles et biomédicales, d'entretiens et d'une enquête sociodémographique, d'étudier la trajectoire de ce traitement en France en tant qu'histoire de médicalisation genrée du vieillissement et exemple de régulation du médicament.

Dissertation title: The trajectory of menopausal hormone therapy in France (1950 - 2020) : between medicalisation and demedicalisation

Menopausal hormone therapy is a medical practice whose relevance and usefulness have ebbed and flowed. Widely dispensed in the 1960’s, it has come under fire from American feminism in the context of criticism leveled against the contraceptive, estrogen pill during the following decade, before reemerging thanks to a new formulation as well as new medical arguments throughout the 80’s and 90’s. Prescribed to almost on in two postmenopausal women at the turn of the century in the US, the crisis of 2002 due to the results of the Women’s Health Initiative led to a change in recommendations stemming from various public health actors and a long-term transnational decrease in its consumption. In the line of works on French history of birth control and gynaecological organisation, based on biomedical and industrial archives in addition to a field study, this thesis aims to study this treatment’s trajectory as a gendered history of the medicalisation of aging and an example of drug regulation.

Titre de la thèse :  Questions de santé dans la nébuleuse écologiste en France depuis les années 1960

L’objectif de cette thèse est de réaliser une histoire intellectuelle et sociale de la santé comme sujet de préoccupation et de discussion écologiste privilégié, depuis les tous débuts de l’écologie politique en France dans les années 60. Dans une perspective socio-historique, ce travail tente de rendre compte des engagements pris sur les fronts conjoints de la santé et de l’écologie dans certains mondes écologistes actifs entre les années 60 et 2000. Il interroge celles et ceux qui durant leurs parcours biographiques ont vécu des formes d’engagements collectifs écologistes (mobilisations locales, journalisme, publications, associations, partis, etc.) dirigés vers la santé humaine. Il s’intéresse également à certains penseurs pionniers ayant écrit sur la santé comme André Gorz, Ivan Illich, Pierre Fournier, et Alexandre Grothendieck, et aux revues d’écologies politiques des années 70-80 (La Gueule Ouverte, Survivre & Vivre, Ecologie, Le Sauvage, Silence etc.) qui rendent compte des débats propres à cette période : entre critique du devenir industriel – et iatrogène – de la médecine moderne, influence de l’environnement (et de l’alimentation) sur la santé humaine, défense des médecines alternatives/naturelles, etc. Aussi, si le recours aux médecines alternatives est régulièrement interprété par les SHS comme comprenant un certain nombre d’enjeux écologiques, il n’existe pas de recherche spécifique et exhaustive consacrée à la place de ce choix thérapeutique dans les engagements écologiques. Cette thèse pose cette question en interrogeant les écologistes engagé.e.s pour la santé à propos du pluralisme thérapeutique : des scènes de débats internes apparaissent et donnent à voir des conflictualités profondes (rapport au corps, à la vie, à la science, à l’expertise, à la liberté thérapeutique…), qui sont d’autant plus d’entraves dans la formulation d’un projet politique écologiste solide et stable. Enfin, cette thèse fait le choix de réaliser un focus sur le recours aux pratiques de santé naturelles en proposant d’interroger un continuum entre ce recours et l’engagement écologique à travers le concept de « sobriété médicale » qui sera évalué par une enquête ethnocomptable des pratiques de santé auprès de quelques foyers.

Dissertation title: The exploration of health issues in ecologist circles in France since the 1960s

The aim of this thesis is to provide an intellectual and social history of health as an important subject of ecological concern and discussion, from the very beginnings of political ecology in France in the 1960s. From a socio-historical perspective, this work attempts to account for the involvements made on the joint fronts of health and ecology in certain ecologist worlds active between the 1960s and 2000. It questions those who, during their biographical careers, experienced forms of collective ecologist commitment (local mobilizations, journalism, publications, associations, parties, etc.) directed towards human health. It also looks at some of the pioneering thinkers who wrote about health, such as André Gorz, Ivan Illich, Pierre Fournier and Alexandre Grothendieck, and at the political ecology magazines of the 70s and 80s (La Gueule Ouverte, Survivre & Vivre, Ecologie, Le Sauvage, Silence etc. ), which reflect the debates specific to this period: between criticism of the industrial - and iatrogenic - development of modern medicine, the influence of the environment (and of food) on human health, the defense of alternative/natural medicines, and so on. So, while the use of alternative medicines is regularly interpreted by the SHS as including a certain number of ecological issues, there is no specific and exhaustive research devoted to the place of this therapeutic choice in ecological commitments. This thesis addresses this research gap by questioning ecologists committed to health on the subject of therapeutic pluralism: scenes of internal debate emerge, revealing deep-seated conflicts (relationship to the body, to life, to science, to expertise, to therapeutic freedom...), which are all the more obstacles to the formulation of a solid and stable ecologist political project. Finally, this thesis focuses on the use of natural health practices, proposing to explore a continuum between this use and ecological commitment through the concept of "medical sobriety", which will be evaluated by an ethnocountable survey of health practices in a number of households.
